What to expect and what to bring, so your appointment goes smoothly.
Before your appointment
- Complete the general registration form (PDF), or the EMG/NCS intake form (PDF) if you are coming for nerve testing.
- Check whether your insurance plan requires a referral. Call 703.664.7285 if you're not sure we accept your plan. See the plans we accept.
What to bring
- Photo ID and your insurance card(s)
- Your completed registration form
- A list of your current medications and doses
- Any referral, imaging reports, or test results related to your visit
- Your copayment. It is due at the time of service.

- Inova Mount Vernon Hospital, 2501 Parkers Lane, Alexandria, VA 22306 (opens Google Maps in a new tab). Phone 703.664.7285. Monday–Friday, 8:00 am–3:30 pm. Use the Blue Parking Lot. The first-floor Rehabilitation Center is next to the green lobby.
After your visit
